Minimalistic Medical Room
Insurance & Medical Reimbursement

Massage therapy may be eligible for reimbursement depending on your insurance plan and medical necessity.

I do not bill insurance directly (at this time).  Payment is due at the time of service.  I provide documentation to support clients who choose to submit claims independently.

What I Provide

Upon request, you may receive:

  • A detailed receipt with provider information

  • Date and duration of service broken down into billable "units" if required

  • CPT and / or ICD-10 codes when applicable

  • Session notes (SOAP notes) if required.

Documentation is accurate, professional, and compliant with Missouri licensure standards.

What You're Responsible For

Before your appointment, contact your insurance provider and ask:

  • Is massage therapy covered under my plan?

  • Do I need a referral or prescription? (with or without diagnostic codes)

  • Are there provider requirements?  (If so, let me know!)

  • What documentation is required?

  • Are there unit and / or visit limits?

Each policy is different.  reimbursement is not guaranteed.

Important Information

  • I do not diagnose medical conditions or create diagnostic codes.

  • I practice within the scope of massage therapy licensure.

  • Coverage decisions are made solely by your insurance provider

  • Payment is due at time of service

If you have questions about documentation prior to booking, feel free to reach out.
